Durable Power of Attorney
A durable power of attorney continues in effect or sometimes takes effect when the principal party is incapacitated or disabled. Durable power of attorney forms available for download: general power of attorney for financial matters and health care power of attorney (health care proxy) for advance medical directives.

A durable health care power of attorney or health care proxy allows an individual, the principal, to grant another person, the proxy, the right and authority to handle matters related to the medical care of the principal. A durable health care power of attorney is also an
advance directive. An advance directive is a document giving direction to health care providers about treatment choices. Advance directives allow the principal to convey his/her decisions, in advance, about end-of-life care. There are two types of advance directives: living will and health care power of attorney.
