General Information:
Durable New Jersey health care power of attorney can become effective upon signing, subsequent disability or incapacity. This durable NJ poa form is often referred to as a health care proxy or medical power of attorney.
Health care power of attorney forms provide specific instructions and designate a health care proxy to make health care decisions when the principal is unable to make them himself / herself.
This New Jersey power of attorney is also an advance directive. New Jersey advance directives convey decisions, in advance, about end-of-life care. A living will is another type of advance directive.
Health Care POA vs Living Will: Both allow advance decisions regarding health care, however, the health care power of attorney also designates a person to serve as proxy (health care proxy) to make medical decisions when there are unanticipated events.
